商業(yè)文件及辦-理藥品、醫(yī)療騎械、保/健/品、化妝品類認(rèn)證所需材料：

辦-理商業(yè)文件所需材料：

1、      中國公司予以印尼合作公司的委托信、代理信或授權(quán)書（注明詳細(xì)地址），并附出口印尼產(chǎn)品的詳細(xì)資料及印尼公司的營業(yè)執(zhí)照復(fù)印件。

2、       自由銷售證/書、產(chǎn)品成分證明、經(jīng)營許可。

3、       中國公司的營業(yè)執(zhí)照。

辦-理藥品、醫(yī)療器械、保/健/品、化妝品類認(rèn)證所需材料:

1、      中華人民共和國藥品、保-健-食-品生產(chǎn)許*可*證（市級以上藥品、食品監(jiān)督管理局簽署）

2、      中華人民共和國藥品、保-健-食-品銷售證明書（市級以上藥品、食品監(jiān)督管理局簽署）

3、       中華人民共和國藥品GMP證/書

4、       企業(yè)法人營業(yè)執(zhí)照

5、       中方公司授予印尼方的授權(quán)書

6、       授權(quán)書中所涉及的產(chǎn)口成分的說明（醫(yī)療器械類產(chǎn)品除外）

上述材料均需由中國國際貿(mào)易促進(jìn)委-員會公證，或在當(dāng)?shù)毓C部門公證；公證后的文件須由中華人民共和國外交部領(lǐng)事司認(rèn)證處認(rèn)證；送交使館認(rèn)證的公證/書需附一份復(fù)印件以及被授權(quán)的印尼公司的營業(yè)執(zhí)照的復(fù)印件。如其中某項已在印尼駐華使館認(rèn)證過，須將其復(fù)印。

 Commercial documents are related to the company'sbusiness, mainly including:

(1) Various commercial documents used for customs clearance andsettlement, such as: certificate of origin, commercial certificate,packing list, bill of lading, commodity inspection certificate,slaughter certificate, ship certificate, insurance policy,etc.;

(2) Various types of certification documents used inforeign-related business activities, such as: contracts, customsdeclarations, authorizations, agency agreements, business licenses,credit certificates, bidding documents, etc.

(3) Other documents related to foreign-related business-relatedactivities that require office-consular certification.

Commercial document certification process: Some commercialdocuments need to be made in the form of commercial certificates,and some can be notarized/certified, and then sent to the agencyfor certification by the Ministry of Foreign Affairs and theEmbassy.

商業(yè)類文件是涉及公司業(yè)務(wù)的內(nèi)容，主要有：

（1）用于通關(guān)結(jié)匯的各類商業(yè)文件，如：原產(chǎn)地證、商業(yè)Invioce、 裝箱單、提單、商檢證、屠宰證、船證明、保險單等；

（2）用于涉外商貿(mào)活動的各類證明文件，如：合同、報關(guān)單、授權(quán)書、代理協(xié)議、營業(yè)執(zhí)照、資信證明、投標(biāo)文件等

（3）其它與涉外商貿(mào)活動相關(guān)需辦-理領(lǐng)事認(rèn)證的文件。

商業(yè)文件認(rèn)證流程：商業(yè)文件有些要做成商事證明書的形式，有些可以做成公證/書，然后送代理處遞交外交部和使館認(rèn)證。

 The documents required for the certification of theembassy and consulate are as follows:

1. Submit one original of the "Consular CertificationApplication Form" (1) and "Consular Certification Application Form"(2);

2. Submit a full set of documents that require CCPITcertification (all embassy certifications must first be certifiedby CCPIT);

3. Submit two copies of the documents to be certified and theircopies.

4. Individual embassies and consulates with special regulationsshall be handled in accordance with their special regulations;

辦-理使領(lǐng)館認(rèn)證需提供的資料如下：

1．提交《領(lǐng)事認(rèn)證申辦表》（1）和《領(lǐng)事認(rèn)證申辦表》（2）各一份正本；

2．提交需經(jīng)貿(mào)促會認(rèn)證的全套資料（所有的使館認(rèn)證必須先經(jīng)貿(mào)促會認(rèn)證）；

3．提交需認(rèn)證的文件及其復(fù)印件二份。

4、個別有特殊規(guī)定的使領(lǐng)館按其特殊規(guī)定辦-理；
